<p><strong><strong>Codebooks</strong><br />
</strong>The integration mainly concerns the databases in which wards, specialists and disciplines are present (Thesaurus 5). The codes used by this database are functional throughout the entire HIS and all connected systems as Centrasys, CDR and Afspraken V5. The HIS codes that formerly consisted out of three digits, were now converted to a four digits combination.</p>
<p><strong><strong>Undisturbed</strong><strong> care process</strong><strong> </strong><br />
</strong>The changing of the codebooks concerned over twenty systems that are linked to the HIS, after which the users should be able to generate and mutate data as usual: printing it on letters, labels and bracelets etc. Since the regular care process was not to be disturbed, the whole conversion took place during the weekend and the night. In the months to follow, more integration phases will take place. But the conversion of the codebooks was the first, the largest and most complicated of the series at the Tergooiziekenhuizen.</p>

<p>The implementation of the iSOFT Afsprakenportaal BROKA fits in the hospital’s strategic targets to upgrade the service level for its patients. After the implementation of BROKA, the product will also be connected to ZorgDomein, creating a direct bridge to referring GP’s. In the third phase, the iSOFT Patient Service Portal will be implemented, by which patients can register themselves online and make appointments directly.<span id="more-328"></span></p>
<p><strong>Easy accessible Solutions</strong><br />
With the Patient Service Portal, patients can also provide information to the hospital online, such as anamneses forms in preparation of a consult or diabetic related data. By providing this service portal, the Vlietland hospital breaks down access barriers for both patient and referring GP’s. The solutions are designed to gain time for all parties involved, because it reduces the duration of the hospital visit and optimizes the healthcare chain. Where necessary, internal processes will be supported and simplified by this solution.</p>

<p><strong>How does BROKA work?</strong><br />
When patients have been referred by a GP, they can take direct active part in the continuation of their treatment. By making use of the by the Vlietland Ziekenhuis offered service, they can plan and reschedule appointments themselves. This directly increases the internal workload because the appointment is no longer made by the registration desk of the hospital. In reaction, patient satisfactory will grow by this fast and accurate planning procedure.</p>
<p><strong>Direct connection with GP’s</strong><br />
The internet application BROKA is part of the iSOFT Afsprakensuite (Appointments). BROKA exists of a relegation portal – designed for online appointment planning and referral by GP’s – and a patient portal that enables patients to make online appointments 24/7. BROKA also forms an important role in the integration of GP’s whom refer patients via ZorgDomein and the appointment-application Afspraken V5, which is part of the hospital.</p>

Visitors can look and experience demo’s of the Lorenzo-EPD, Multi Media Sharing-XDS in combination with Mirador, MicroHis X and demo’s of several Connectors, under which the Connector Patiëntinschrijving (Patient registration). Also, iSOFT will demonstrate the connection to the National EPD (Electronic Patient Record) and other pharmaceutical solutions.</p>
<p>The various demo’s follow four Lines: GP/Hospital, Patient/Hospital, Hospital/Pharmacy and the connection to the National EPD. The first line applies to the data-exchange from hospital to the GP’s. Through the Lorenzo-EPD viewer, GP’s are able to have access to the patients record in the hospital. With MMS-XDS, GP’s can view and review photographic material made in the hospital, such as the patient’s ECG scan.<span id="more-325"></span></p>
<p><strong>Demo’s Patient logistics</strong><br />
In addition, iSOFT demonstrates several solutions for patient logistics. Patients whom are forwarded to a hospital by their GP, will henceforth be able to register themselves online, taking direct part in the process of their treatment. The solution Patient Service Portal, which is made available by the hospital, also enables the patient to make appointments with specific inter-hospital departments or doctors. The Connector Patiëntinschrijving (patient registration) verifies the inserted data when the patient arrives for the appointment. With the picture-functionality, the registration office can easily scan the patient’s ID and attach the document to the EPD. The ID will also directly be processed for online verification.</p>
<p><strong>Connection EPD and Pharmacy</strong></p>
<p>iSOFT also demonstrates that data-exchange is proceeded safely between treating hospital staff and the pharmacy’s connected to the GP’s office. With Medication/TMV, the hospital doctor has insight in any medication prescribed outside the hospital to a patient. This way, errors in conflicting medication can be prevented. This contributes directly to the patients safety. Finally, an additional demo demonstrates how iSOFT connects its hospital to the National EPD.</p>
